AT Texas Health Cleburne the Clinical Nurse Leader serves as the point person on patient care teams, and they are leaders in the health care delivery system across all settings in which health care is delivered. JOB REQUIREMENTS:
Master's degree in Nursing; Clinical Nurse Leader degree
Three years of current hospital experience and two year in a clinical specialty.
Current Texas RN License
Duties:
The Clinical Nurse Leader oversees the lateral integration of care for a distinct group of patients and may actively provide direct patient care in complex situations. This position will be unit based, supervising a small cohort of patients based on the 12 to 16 bed model. They put evidence-based practice into action to ensure that the patient benefit from the latest innovations in care delivery. This leader collects and evaluation patient outcomes, assesses cohort risk, and has the decision-making authority to change care plans when necessary. This clinician functions a part of an interprofessional team by communication, planning, and implementing care directly with other health care professions, including physicians, pharmacists, social workers and etc.
Hours: FT 8-5p.
